Capital Health Plan is a health plan with a Medicare contract. The contract is renewed annually and the availability of coverage beyond the current year is not guaranteed.

Benefits, formulary, pharmacy, network premium and/or copayments/coinsurance may change on January 1, 2011. Please contact Capital Health Plan for details.

Please call Capital Health Plan Member Services Department to obtain documents in alternate formats or languages.

To ensure that beneficiaries receive appropriate care, Capital Health Plan will follow policies and procedures as directed by CMS (Centers for Medicare and Medicaid Services) in the event of an emergency situation designated by the Department of Health and Human Services.

Enroll in CHP Retiree Advantage

Please complete the Retiree Advantage enrollment application (pdf 116.70 kB) and contact your employer's benefits administrator to apply for Capital Health Plan Retiree Advantage.

Late Enrollment Penalty

If an individual does not join a Medicare drug plan when he or she first is eligible, and goes without creditable prescription drug coverage (as good as Medicare's) for 63 continuous days or more, the individual may have to pay a late enrollment penalty to join a plan later. This penalty amount changes every year and must be paid as long as the individual has Medicare prescription drug coverage. However, if the individual qualifies for extra help, he or she may not have to pay a late enrollment penalty, or may get help paying the late enrollment penalty.

Please check with your group administrator to determine if your employer-sponsored Capital Health Plan coverage is considered creditable prescription drug coverage and whether you have had any coverage gap for 63 continuous days or more.
